Order Management

The laboratory will perform tests only at the written or electronic request of an authorized institution. Verbal or faxed requests for additional laboratory tests are permitted by the referring institution only .  CPAL can not accept orders directly from patients or physicians.  A referring laboratory may request CPAL to do reflexive testing based on current or previous laboratory results.  Requests for  auto-reflexive testing should be submitted to the Laboratory Information System Manager, Technical Director or Administrative Director.  CPAL will provide CPT-4 codes for each test.

Requests for Test Results

CPAL will provide lab test results via the submitting institution.  The Medical Director, Technical Director and various technical staff are available to assist physicians in ordering the appropriate testing and to supply technical information regarding testing techniques.   Contact CPAL at  (717)851-1416 or (888) 480-1422
  
Faxed Results
Laboratory staff will transmit a fax report when requested by the referring institution.  If a physician requests a fax report, CPAL will fax the report to the referring institution and the referring institution will use its internal policies to forward the reports to the ordering or consulting physician.

Called Results
CPAL will call test results upon specific request at the time the test was ordered.  Results will be called to the phone number supplied with test request.

Repeat Determinations

Repeat determinations of test results  are performed at no charge whenever CPAL’s reported result(s) does not, in the opinion of the physician, fit the clinical condition  of the patient.  The situation  should be explained to a member of the CPAL management staff when requesting repeat testing.  CPAL  will repeat only the same test as originally requested.

Research
CPAL will  participate in independent research.  If a research project is anticipated, the full details of the project should be discussed with  CPAL’s Medical Director or Administrative Director.  The laboratory administrative group will determine CPAL’s capabilities to participate in the research project and establish any special arrangements, services or fees required.

Record Retention

Electronic blood donor and patient records are retained in the live computer environment for forty-five days following completion.  After forty-five days they are purged to a tape backup system.

Paper copies of  patient testing records are retained for two years.  Donor testing records are stored indefinitely.

Serum, plasma, whole blood specimens: 7 days refrigerated
PCR specimens: 1 day refrigerated after reported
Electrophoresis specimens: 14 days refrigerated
Blood Donor Specimens: 7 days refrigerated
Positive HCV IgG Ab, HIV ½ Ab, HBsAG, HBcAB Poly, HTLV I\II Ab: 1 year at -18oC or colder
HCV Look back: 1 year at -30oC
Positive Lyme IgG/IgM Ab Screens: 14 days at -18oC or colder
Thin Prep Vials: 3 months from receive date at RT

Result Reporting

Routine Reporting 
Results are transmitted back to the referring institution via a computer interface and, when appropriate,  results can be retransmitted.  To have a result re-transmitted please call the CPAL LIS Manager or a CPAL staff member.  For clients who are not interfaced to CPAL, orders and results can be provided using a CRT and printer at the client site.

Fax Reports 
A paper copy of  test results can be requested.  To receive a fax copy, please call the CPAL LIS Manager or a CPAL staff member.

Reference Laboratory Reports 
The referring institution will receive a copy of  any testing sent to a reference laboratory.  This report may be sent to the referring institution directly from the reference laboratory or by CPAL.

Computer Codes Used on Reports

* -- An asterisk indicates an abnormal alpha result.  The asterisk will only appear if the procedure has an established normal range.

H -- An upper case H indicates a numeric value higher than the established reference range.  The H will only appear if the procedure has an established normal range.

L -- An upper case L indicates a numeric value lower than the established reference range. The L will only appear if the procedure has an established normal range.

C -- An upper case C indicates a critical value.  The C will only appear if the procedure has  established limits for critical high or low values.

c -- A  lower case c indicates a corrected result.  A footnote will provide the previous value,  date and time of correction.  Note:  all corrected reports are phoned and faxed to the referring institution. 

f -- A lower case f  indicates a footnote associated with the test or result.

ND -- Not done; a report will not be forthcoming, and the charge has been canceled.  

QNS -- Quantity of the specimen is not sufficient for testing; a report will not be forthcoming, and the charge has been canceled.

@ -- Denotes test was performed at a reference laboratory.  A footnote will indicate the reference laboratory’s name.

Note: In most cases, any abnormal result will also print in bold type.

Reference Laboratories

Laboratory procedures not performed at CPAL  will be sent to an approved reference laboratory.  Each laboratory has been approved by either the Laboratory Advisory Board or the appropriate CPAL Affinity Group.  The reference laboratory will bill the referring institution directly.  CPAL will not pay for reference laboratory testing requested by a member institution.

Blood Collection Tubes

GOLD TOP GEL (Serum Separator Plastic Tube)

Sterile plastic serum separator tube.  Contains a gel substance at bottom of tube and clot activator on interior wall of tube.  No anticoagulant.  Available in 13 x 100 with 5 ml  volume draw.  Invert at least 5 times to facilitate mixing.  Allow  blood to clot  for 30 minutes and centrifuge for 10 minutes at 2300 RPMs.  After centrifugation, the gel forms a barrier between the clot and the serum.  

LAVENDER TOP (PLASMA Plastic Tube)

Sterile plastic plasma tube.  Contains Potassium EDTA as anticoagulant.  Available in 13 x 75 (3ml) or         13 x 100 (5mL or 7 ml) volume draw.  Invert several times to facilitate mixing and prevent coagulation.      

RED TOP

Sterile plastic serum tube.  Contains no additive.  Available in 13 x 75 (3ml) or 13 x 100ml (7ML)volume draw.  Allow  blood to clot  for 30 minutes and centrifuge for 10 minutes at 2300 RPMs.  

RED/GRAY TOP GLASS GEL (Serum Separator Glass Tube)

Urine Cup

Plastic screw cap cup. Contains no additive.  Available in 50ml volume.  Place urine into cup and store at 2-8oC.
